To protect our most vulnerable community members and to ensure a fall semester with as few restrictions as possible, Salve Regina is requiring the COVID-19 vaccine for the 2021-2022 academic year.

All members of the Salve Regina community – including resident students, off-campus/commuter students, graduate students, full- and part-time staff, full-time and adjunct faculty and athletic coaches – are required to be fully vaccinated unless they are approved for a medical or religious exemption.

Salve Regina considers individuals to be fully vaccinated two weeks after their second dose of Pfizer or Moderna or two weeks after the single-dose of Johnson & Johnson (Janssen) vaccine. A WHO-approved vaccine is acceptable for international students.

Undergraduate and Graduate Students

To submit your vaccination record:

  • Log in to the health portal with your Salve Regina credentials.
  • In the "My Forms" menu at the top of the page, select and complete the "COVID vaccination form."

Health Services will assess student requests for medical and religious exemptions on a case-by-case basis.

International students who are unable to access a WHO-approved vaccine prior to returning to the United States will be asked to vaccinate upon return to campus. Please click here for more details.

The vaccine requirement applies to graduate students who are taking in-person classes and/or need to access campus facilities during the academic year. Graduate and RN-BSN students who are fully online and have no need to access campus resources do not need to submit proof of vaccination.

Faculty and Staff

You may provide proof of vaccination securely and confidentially via the employee submission form. No other medical information should be provided.

Medical and religious exemptions will be considered on an individual basis. Please use the exemption request form to request an exemption based on a bona fide medical or religious reason. Forms should be printed, completed and returned the Office of Human Resources via mail or in person to ensure confidentiality.
