If you do not wish to apply online, a PDF version of the application is available below that can be printed and mailed (along with the additional required materials) to the office of Graduate Studies and Continuing Education.

All candidates must furnish the following:
- Completed application with the non-refundable $25.00 application fee.
- Official transcripts from all degree-granting institutions.
- A one page, word processed personal statement discussing your decision to return to school.
- Two (2) letters of recommendation from individuals who are positioned to evaluate the applicant's academic ability and commitment to program requirements.
