| Student-Athlete Information
All student-athletes must provide the following completed forms:
YOU MUST BE MEDICALLY CLEARED WITH AN ANNUAL PHYSICAL BEFORE YOU BEGIN VARSITY PRACTICES EACH SEASON AT SALVE REGINA UNIVERSITY.
Please print legibly with blue or black ink and sign all forms where indicated. Note: Acrobat Reader is needed to view/print forms (click here if you want to download application).
- Athletic Participation Physical Form - please mark the appropriate answer to each question. If you have answered " Yes" to any questions, please provide an explanation.
- For non-Rhode Islanders, please utilize the HMO Referral Form to expedite any treatment necessary from the Salve Regina Sports Medicine Staff.
- If you have had any significant injury or surgery in the past 12 months, please include a written release from the treating physician stating you are cleared to participate in intercollegiate athletics.
- If you are under the age of 18, a parent or legal guardian must sign all forms.
DEADLINES
- If you participate in a Fall Sport (cross country, field hockey, football, sailing, soccer, women's tennis, women's volleyball), your completed paperwork must be received by 15 July 2007.
- If you participate in a Spring Sport (lacrosse, baseball, softball, men's tennis, women's track & field), your completed paper work must be received by 15 August 2007.
- If you participate in a Winter Sport (basketball, ice hockey), your completed paperwork must be received by 1 September 2007.
Please make and retain a copy of all forms and mail (faxes will not be accepted) the ORIGINALS to:

Athletic Training Room Rules
- Report all injuries to the athletic trainer as soon as possible. This includes non-athletic injuries.
- All athletes must sign-in prior to receiving any treatments.
- Check with Athletic Training Staff before any treatment. This includes heat and ice.
- It is the athlete's responsibility to get to the ATR with enough time to get their treatment and do their rehabilitation program before practice.
- Treatments, rehabilitation and evaluation are done by appointments only.
- Do not bring equipment, bags, cleats, etc., into the ATR.
- No eating or drinking in the ATR.
- Once you are injured or sick, you must be cleared by the athletic training staff before you can return to practice or games. No exceptions!
- Ask permission before taking any supplies out of the ATR.
- Please return all ace wraps, neoprene sleeves, or special equipment to the ATR when they are no longer needed. If not returned, you will be charged for the replacement cost.
